INSTRUCTIONS
1) Pick An Amount ($10, $25, $50, $100 or $250)
2) Write in the Recipient's Name (This is REQUIRED)
3) If you want US to notify the recipient by EMAIL, then Check the box that says "Send E-Mail Notice to Recipient?" (This is a free service)
4) If you checked the box in #3 above, then be sure to write their Email address in this box.
5) If you want a note to be displayed in your email, then write it in the "Note to Recipient" field.
